Files and forms for enrollment to WCR – updated April 2020
Please complete and submit the following forms in the Enrollment Packet:
- Emergency Contact Form
- 7th OR 8th Grade elective Form
- Enrollment Form
- Cumulative Records Release Form
- Health History Form
- Residence Verification Form
- Parent’s Rights Form (Completed Last Page)
- CJUSD Rules for Accepted Use of Computers and Computer Networks
- Residence Questionnaire (if applicable)
Enrollment packet is at the bottom of the page.
Also required for registration:
- Student’s current shot record
- Copy of last report card
- 1 Form of proof of residency in the district (Utility bill such as SMUD or PG&E)
- Checkout from previous school
- Birth Certificate
Transportation
If you student needs to take the bus to school for the school year, please visit this site for info and to fill out the form.
